Caro diario is divided into three chapters: ‘On My Vespa’, ‘Islands’ and ‘Doctors’. The first part is set in Rome, where Moretti visits his favourite neighbourhood Garbetella. “Wouldn’t it be wonderful to make a film with only pans of houses?” he wonders. In part two, he visits a friend on the island of Lipari who has studied James Joyce’s Ulysses for eleven years and hasn’t watched television for thirty.

In the third part, Moretti films his own chemotherapy and tries various other cures and therapies. Caro diario is a heterogeneous yet coherent film, even in Moretti’s eyes, and he has said that the three parts together form an archipelago with separate sections as islands: the neighbourhoods of Rome, the Aeolian islands and the doctors isolated in their specialisations and unable to communicate with each other or with their patients.
